I am posting this request as a matter of making this document a matter of public domain. The reason I have a blog is to catalogue the events in the history of my injury, so I can remember later what happened. As it turns out, I have referred back to my old posts many times in order to reference what precisely transpired. It has been a very useful tool in writing current content and the recall of documents, etc.
